In order to streamline the intake process, we have uploaded some of the Arete intake forms to our site. Some clients prefer to fill out these forms in advance to simplify the process, but this is entirely optional and a matter of client preference. If you do not wish to fill out these forms in advance, they can also serve as good general information to begin thinking about the kind of questions that will come up in therapy.

Most PPO and POS insurances accepted as reimbursable under the out-of-network fee schedule and out-of-pocket expenses may count toward your annual deductible. Treatment is generally a qualified expense under most Flexible Spending Accounts (FSA), Health Savings Accounts (HSA), and Health Reimbursement Accounts (HRA). Contact your insurance carrier for details.
